SMS Terms of Service

These SMS Terms of Service ("SMS Terms of Service") governs the manner in which PatientIQ (“PatientIQ,” “we,” “our,” “us” and similar terms) collects, uses, maintains and discloses information collected from users (a “user,” as well as “you,” “your” and similar terms) of our SMS services (“SMS Services”).

Program description

Our text messages are intended to provide you with information related to your health care. The name of this text messaging program is Health Messages.

Message frequency

Message frequencies will vary depending on your health care needs.

How to opt-in

To opt-in to receive PatientIQ Health Care Messages, please follow the instructions and provide the required information after you log into the PatientIQ patient portal. Or you may email us and receive instructions to opt-in.

How to opt-out

You can cancel the SMS Services at any time by texting “STOP”. When you send the SMS message “STOP” to us, we reply with an SMS message that confirms that you have been unsubscribed. After this, you won’t receive any additional SMS messages from us. If you want to join again, sign up as you did the first time or contact us, and we will start sending SMS messages to you again.

Cost

Message and data rates may apply for any messages sent to you from us and to us from you. Contact your wireless provider for more information about your text plan or data plan. We will not charge you for sending you text messages.

Your mobile telephone number
You represent that you are the account holder for the mobile telephone number(s) that you provide. You are responsible for notifying us immediately if you change your mobile telephone number. Please contact us to update your information.
You agree to indemnify PatientIQ in full for all claims, expenses, and damages related to or caused in whole or in part by your failure to notify PatientIQ if you change your telephone number, including, but not limited to, all claims, expenses, and damages related to or arising under the Telephone Consumer Protection Act.

You can get more information at any time by texting “HELP”. When you send the SMS message “HELP” to us, we will respond with instructions on how to use our service and how to unsubscribe. You can also email us at: support@patientiq.io

Privacy

If you have any questions regarding privacy, please read our Privacy Policy. Data obtained through the Health Messages program will not be shared with any third-parties for their marketing reasons/purposes

Supported carriers
We are able to deliver messages to the following mobile phone carriers: Major carriers: AT&T, Verizon Wireless, Sprint, and T-Mobile USA. The supported carriers my change from time to time.
Carriers are not liable for delayed or undelivered messages. PatientIQ is not liable for delayed or undelivered messages.
 
Service is not guaranteed

Delivery of information and content to a mobile device may fail due to a variety of circumstances or conditions. You understand and acknowledge that network services, including but not limited to mobile network services, are outside of PatientIQ's control, and PatientIQ is not responsible or liable for issues arising from them.

Eligibility

You must be a resident of the United States and 18 years of age or older to receive SMS Services. PatientIQ reserves the right to require you to prove that you are at least 18 years of age.

Changes to terms and conditions

PatientIQ may revise, modify, or amend these SMS Terms of Service at any time. Any such revision, modification, or amendment shall take effect when it is posted to PatientIQ’s website. You agree to review these SMS Terms and Service periodically to ensure that you are aware of any changes. Your continued consent to receive PatientIQ text messages will indicate your acceptance of those changes.

Termination of text messaging

PatientIQ may suspend or terminate your receipt of PatientIQ text messages if PatientIQ believes you are in breach of these SMS Terms of Service. Your receipt of PatientIQ text messages is also subject to termination in the event that your mobile telephone service terminates or lapses. PatientIQ reserves the right to modify or discontinue, temporarily or permanently, all or any part of PatientIQ text messages, with or without notice.
