Description: The Oswestry Disability Index (ODI) is one of the most commonly used tools to measure a patient's permanent functional disability. It is specifically designed for patients with low back pain to determine how the condition affects their ability to manage in everyday life. The ODI evaluates the impact of back pain on activities of daily living including personal care, lifting, walking, sitting, standing, sleeping, social life, traveling, and employment/homemaking.

Number of times: 10

Estimated time to complete:3-5 minutes

Scoring Method:

Each of the 10 items is scored on a 6-point scale from 0 (no disability) to 5 (maximum disability). The scores for each section are added together, divided by the total possible score (50 if all questions are answered), and then multiplied by 100 to yield a percentage. A higher score indicates a higher level of disability:

  • 0–20%: Minimal disability

  • 21–40%: Moderate disability

  • 41–60%: Severe disability

  • 61–80%: Crippling back pain

  • 81–100%: Either bed-bound or exaggerating symptoms
