How Panorama Orthopedics & Spine Center Proves Leadership in Value-Based Care

Panorama Orthopedics & Spine Center is partnering with PatientIQ, a Chicago-based healthcare software company, to launch value-based care reimbursement programs for leading insurance providers using PatientIQ’s cloud-based patient engagement and analytics platform. 

These programs will analyze data aggregated from NextGen EHR and Panorama’s other clinical databases to advance value-based care and the quadruple aim of providing better quality care for patients, improving population health management, reducing the cost of care, and optimizing clinical workflows.  

Care teams at Panorama will use PatientIQ’s EHR-integrated workflows to automatically enroll patients into custom care pathways for total knee arthroplasty, total hip arthroplasty, lumbar laminectomy, or lumbar fusion. From the first preoperative visit through ninety days post-operatively, Panorama will deliver targeted education, pre-rehabilitation modules, and assessments to patients. The platform will also risk stratify patients, generate real-time actionable insights to improve care coordination, and produce quality reports for monitoring performance.  

Integrated closely with outpatient physical therapy and rehabilitation services, care navigators at Panorama will leverage the PatientIQ dashboards to monitor every patient enrolled in the value-based reimbursement program. These programs ensure patients receive highly personalized care to meet their individual needs and optimize them for the best possible outcome.
